Cancellation Policy

We request at least 24 hours' notice when canceling or rescheduling an appointment.

A late cancellation or no-show fee will apply when an appointment is canceled with less than 24 hours' notice or missed without appropriate notice, consistent with our current practice policies and applicable agreements.

We understand that genuine emergencies can occur and may consider emergency circumstances on a case-by-case basis.

INSURANCE AND PAYMENT TERMS

Insurance

Maple Leaf Counseling, Inc. DOES NOT accept insurance plans. Maple Leaf Counseling, Inc. has partnered with Mentaya to help clients use their out-of-network benefits to save money on therapy.

Although we make reasonable efforts to verify benefits and coverage information, verification does not guarantee that an insurance company will pay for services.

You are responsible for understanding your insurance benefits, deductibles, copayments, coinsurance, exclusions, and other applicable financial responsibilities.

Payment

Payment is generally due at the time services are provided unless other arrangements have been approved in advance.

We accept Credit Cards, Checks, HSA/FSA Cards. You are responsible for the full balance to be paid.

Outstanding balances may be subject to collection procedures consistent with applicable law and our practice policies.

TERMINATION OF SERVICES AND WEBSITE ACCESS

You may stop using our website or discontinue services at any time.

We may suspend or terminate access to our website or online systems when we reasonably believe that a user has violated these Terms of Service, applicable law, or our practice policies.
